Corticosteroids for the prevention of ventilator-induced lung injury?

چکیده
1. Departamento de Medicina Clínica, Universidade Federal do Ceará, Fortaleza (CE) Brasil. Numerous pharmacological therapies for acute respiratory distress syndrome (ARDS) have failed to demonstrate a benefit in multicenter clinical trials.(1) Given that dysregulated inflammation is a prominent feature of ARDS, systemic corticosteroids are thought to represent a potentially beneficial therapy.(2)

Physical and biological triggers of ventilator-induced lung injury and its prevention.
Ventilator-induced lung injury is a side-effect of mechanical ventilation. Its prevention or attenuation implies knowledge of the sequence of events that lead from mechanical stress to lung inflammation and stress at rupture. A literature review was undertaken which focused on the link between the mechanical forces in the diseased lung and the resulting inflammation/rupture. The distending forc...
متن کاملReducing ventilator-induced lung injury and other organ injury by the prone position
Mechanical ventilation can cause structural and functional disturbances in the lung, as well as other vital organ dysfunctions. Apoptosis is thought to be a histological sign of distant organ damage in ventilator-induced lung injury (VILI). Nakos and colleagues observed a protective effect of prone positioning against VILI in normal sheep. Less alteration in the lung architecture and function a...

Despite the recent advances in the care of critically ill patients,1 ARDS is still a clinical condition associated with high mortality rates. In this clinical scenario, while most interventions to date have focused on the prevention of morbidity with ventilator-induced lung injury and pneumonia, no therapeutic intervention is indisputably associated with improved outcomes. Corticosteroids have ...
متن کاملVentilator-associated lung injury.
Mechanical ventilation of disease-affected lungs, as well as being an inadequate mode of ventilation for initially healthy lungs, can cause significant changes in their structure and function. In order to differentiate these processes, two terms are used: ventilator-associated lung injury (VALI) and ventilator-induced lung injury (VILI). In both cases, lung injury primarily results from differe...
متن کاملAutophagy facilitates ventilator-induced lung injury partly through activation of NF-κB pathway
Mechanical ventilation is an important supportive therapy in the intensive care unit (ICU) to assist the critically ill patients with respiratory failure. But longer ventilation time has been proven to contribute to the lung injury which has been recognized as ventilator-induced lung injury (VILI). Recently studies have suggested that NF-kappaB signaling pathways may play a critical role in the...
